NEW & PRESCHOOL STUDENTS
All new students at Grace Christian School are required to submit the Physician's Report and Dental Form prior to the start of the school year. In addition, all Preschool students are required to resubmit the Physician's Report and Dental Form yearly.

STUDENTS ENTERING 7th GRADE
Students entering 7th grade are required to submit the TDAP Immunization form prior to the start of the school year.

SPECIAL FORMS
The following forms may be submitted to GCS as needed. These forms must be submitted directly to the school nurse and must accompany the medication referenced in the form.
